Registered Nurse (RN) (Indigenous Health)

Shift: Full Time 9:00am – 5:00pm, (with the possibility of some weekends)

JOB FUNCTION:

The RN position is intended to support the team at Auduzhe Mino Nesewinong in creating a welcoming and culturally safe space for Indigenous clients, and to develop and provide supports. The RN will provide wholistic and comprehensive care for adults and children attending the Auduzhe Mino Nesewinong Indigenous Interdisciplinary Primary Care Team site. This position may require attending outreach clinics or home visits to deliver care to Indigenous clients.

JOB FUNCTION:

To provide confidential case management services including:

  • Addressing the basic needs of clients
  • Providing supports on the street and in various locations throughout the city, assisting clients in an effort to effectively live a life away from homelessness toward a more self-sufficient lifestyle.
  • In an effort to create a continuum of care we offer support and Indigenous Specific Aftercare to people who have made the transition from streets to homes.

JOB FUNCTION:

The Mobile Outreach Worker will provide support to homeless individuals living outdoors on the street, in encampments, parks and ravines. The MOW will address a client’s basic needs to live away from outdoor homelessness and toward a more self-sufficient lifestyle, while ultimately working towards finding safe suitable housing.

JOB FUNCTION:

As a member of the Mino Kaanjigoowin team, the Case Manager provides physical, emotional, mental and spiritual care and support for clients with mental illness, concurrent disorders and special needs in order to assist them to effectively transition from homelessness to stable and secure long-term housing.

JOB FUNCTION:

The Case Manager will provide confidential case management services that address the basic daily needs of clients housed across Nameres housing sites.  They will assist clients to be self-sufficient, while maintaining housing with the resources required to enhance the quality of their lives. The Case Manager will also provide daily supports through traditional teachings, connect clients with community resources, and Elders in the community.
