HOURS: 8am – 4pm
SALARY: $48,000.00
Na-Me-Res (Native Men’s Residence) located in Toronto, is a diverse, multi-service organization. Through a culture-based approach that addresses the holistic needs of its clients Na-Me-Res’ mission is to provide temporary, transitional and permanent housing to Indigenous men experiencing homelessness in Toronto, while providing outreach and support services to the broader Indigenous homeless population.
MAIN FUNCTION: In partnership with the Blue Door Shelters, Na-Me-Res will be expanding their services to the York Region. This role will be housed in the Blue Door Shelter offices and will liaise with the Blue Door Shelter staff to connect with Indigenous clients. The Outreach Worker will provide comprehensive outreach support and case management services to Indigenous people in the York region. This role will address the immediate needs of clients and provide support on the street and at various locations throughout the region. The Outreach Worker will intensively work with clients to obtain and maintain housing while assisting clients to stabilize physically, emotionally, mentally and spiritually.
DUTIES AND RESPONSIBILITIES
- Provide street outreach to a variety of locations throughout the region, visit encampments and connect with Indigenous individuals to provide support and resources
- Provide culturally safe care to Indigenous clients with a focus on mental, emotional, spiritual and physical support
- Facilitate connection to Indigenous culture and community through a variety of traditional methods
- Work closely with the Blue Door Shelter staff to build connections with community service providers with the goal of identifying clients who need support
- Identify type of housing needs including shelter, transitional, supportive, long-term care facilities as they work towards permanent residential setting
- Assist and support clients to complete housing applications, to reactivate existing applications and other applications as required
- Support and accompany client to housing and other appointments and services when appropriate
- In partnership with the client, develop an aftercare plan which includes follow-up services
- Assist client in locating and utilizing community resources as supports to maintain their housing
- Provide eviction prevention and intervention service to clients whose housing security is at risk
- Enter all client information in our client database, as per our funding agreement.
- Participate in regular case management supervision
- Other duties as identified by the Supervisor
SKILLS REQUIRED
- Familiarity with community support services and housing resources for homeless individuals in the York Region
- Ability to prioritize and manage multiple jobs effectively
- Ability to work under pressure and to meet deadlines with minimal supervision
- Able to communicate effectively – good oral and written communication skills
- Knowledge and ability to work with computers and databases
- A valid Ontario Driver’s License and clean driving abstract is a requirement (nine years of driving experience)
EDUCATION & EXPERIENCE
- A degree or diploma in social services or other related discipline
- Minimum two years of related work experience
- Successful experience working with Indigenous communities
- Must pass Vulnerable Sector check
- Successful experience working with a diverse population
- Knowledge of Indigenous traditions and culture is an asset
